Our team has supported people from the beginning of their care journey, helping them move from hospital environments into their own homes, build confidence and enjoy meaningful experiences with the right support in place.

One person we support now owns her own dog and enjoys holidays abroad with her family while continuing to be supported by our team. Another enjoys cruises and day trips into London to watch shows such as The Phantom of the Opera. She also recently ate her first pizza in years, a small but meaningful moment that reflects what person-centred care is all about.

What is a care plan?

A care plan is a personalised, written plan that outlines a person’s health, social and personal care needs, along with the specific support and actions required to meet those needs. Care plans are thoughtfully created by our care coordinators to ensure each individual receives the right level of support.

How is support tailored to the individual?

Our support is always person-centred, which means the people we support are right at the heart of every decision we make. Whether we’re putting together care plans, planning a day out, or organising a holiday, we focus on what matters to each individual, from their needs and preferences to what they want to achieve.

What support can be provided at home?

Our professional team is trained to support individual needs and requirements, including PEG feeding, medication management, personal care, PBS and activities of daily living, to name just a few. Speak to our team to find out how we can support you in the comfort of your own home.

How does support begin?

You will have a chat with our friendly team, who will assess your needs, requirements, likes and dislikes. You will then review and sign your care plan. Your care team will be selected based on shared similarities with you, and your care will begin.

What’s the role of families in complex care packages?

Family involvement in care is a key part of delivering real person-centred support, as it helps improve outcomes by bringing in important insights about a person’s preferences, life history and personal needs. We make sure families are kept informed through regular communication and actively involved in decision making.

What is a care plan?

A care plan is a personalised, written plan that outlines a person’s health, social and personal care needs, along with the specific support and actions required to meet those needs. Care plans are thoughtfully created by our care coordinators to ensure each individual receives the right level of support.

How is support tailored to the individual?

Our support is always person-centred, which means the people we support are right at the heart of every decision we make. Whether we’re putting together care plans, planning a day out, or organising a holiday, we focus on what matters to each individual, from their needs and preferences to what they want to achieve.

What support can be provided at home?

Our professional team is trained to support individual needs and requirements, including PEG feeding, medication management, personal care, PBS and activities of daily living, to name just a few. Speak to our team to find out how we can support you in the comfort of your own home.

How does support begin?

You will have a chat with our friendly team, who will assess your needs, requirements, likes and dislikes. You will then review and sign your care plan. Your care team will be selected based on shared similarities with you, and your care will begin.

What’s the role of families in complex care packages?

Family involvement in care is a key part of delivering real person-centred support, as it helps improve outcomes by bringing in important insights about a person’s preferences, life history and personal needs.
